Supporting service continuity and growth for PCN

Supporting gaps in service provision

At a key stage in their development, Cross Counties PCN experienced an unexpected gap in service provision from a previous provider during the Covid-19 pandemic. This created immediate pressure across the team, particularly in managing clinical correspondence, administrative workload, and patient queries.

The PCN needed a solution that could be implemented quickly, while maintaining continuity and minimising disruption to existing workflows - this is where MMS stepped in to support.

Approach

MMS were able to onboard pharmacists in a very short timeframe to provide pharmacist-led support tailored to the needs of the PCN, and to quickly address the delivery gap that the PCN had encountered. The MMS team integrated quickly into existing ways of working, ensuring that services could continue without interruption during a challenging period.

From an initial short-term support agreement, this has developed into a longer-term partnership, with MMS continuing to support both day-to-day workload and wider service delivery, including long-term condition clinics.

With MMS support in place, Cross Counties PCN has been able to:

  • Maintain service continuity during a critical period
  • Reduce pressure on internal staff
  • Stay on top of clinical correspondence, admin, and patient queries
  • Create capacity to support service growth and development
  • Improve efficiency across the wider team

The partnership has also played a key role in supporting the development of the PCN’s Clinical Pharmacy service, enabling it to expand rather than simply maintaining current delivery.

Across Q1 in 2026, our team delivered:

  • 116 total hours logged on
  • 661 medicines-related tasks
  • 254 hospital discharge/ meds reconciliations
  • 108 structured medication review
  • 6 CQC reviews
