We provide implementation support on delivering key QOF indicators to boost practice income, reduce GP workloads, and improve patient outcomes.

Overview

Our experienced team of pharmacists collaborate with your practice to review QOF targets, identifying specific areas that maximise income and deliver improved patient outcomes. Our team has helped practices meet their targets, identify and support missed coding to generate additional QOF income.

How We Support GP Practices

Delivering QOF-aligned activity
Improve QOF data quality
Reducing GP workloads
Enhancing patient care

Delivering QOF-aligned activity

Through identification of opportunities to managing patient conditions and support achievement of QOF points, our pharmacists can:

  • Conduct therapy‑specific reviews (e.g., diabetes, hypertension, asthma, COPD, heart failure) within scope
  • Support personalised care planning and shared decision-making
  • Implement evidence-based prescribing aligned with NICE guidance
  • Support quality improvement modules through audits and action plans

Improve QOF data quality

MMS’ team of pharmacists work with practices to:

  • Ensure accurate coding of diagnoses, reviews, and clinical measurements
  • Identify missing QOF indicators and proactively recall patients
  • Undertake gap-analysis searches in care (e.g., missing HbA1c, BP, asthma reviews)
  • Ensure QOF achievement is fully captured through data-cleansing

Reducing GP workloads

By delivering key tasks linked to the QOF framework, MMS can support reducing GP workloads by delivering:

  • Medication related tasks such as prescription queries, medicines reconciliation, and Rx reviews
  • Structured medication reviews (SMRs) for complex patients
  • Long-term condition clinics by reviewing treatment optimisation opportunities
  • Monitoring requirements for high risk medicines

Enhancing patient care

Partnering with GP practices and understanding the needs of their patient cohort, we can enhance patient care and outcomes by:

  • Providing patient education on medicines, adherence, and lifestyle
  • Improving safety through deprescribing, monitoring, and risk mitigation
  • Ensuring timely follow up after hospital discharge
  • Supporting vulnerable patients with complex polypharmacy

Enhanced Access

Pharmacists deliver support for enhanced access by running out-of-hours clinics, which include:

  • Chronic disease management clinics for long-term conditions, such as diabetes, hypertension, and cardiovascular disease
  • Respiratory review clinics, including ACT scoring and therapy optimisation
  • Structured Medication Reviews for targeted patient groups, such as those with polypharmacy and high-risk medication
