Medicines Management Solutions are seeking to recruit an independent prescribing clinical pharmacist to support GP workflow within a GP Practice in the Crewe (Cheshire) area.
*Knowledge of EMIS and previous GP Practice experience (1 years) is required for this role. The successful individual also needs to be an independent prescriber. UK Based applicants ONLY with GPhC registration.*
We are looking for a motivated, enthusiastic and professional individual with a good clinical background, who is organised and confident to work independently and as part of a team, with the ability to prioritise workload. Applicants must be computer literate and have excellent communication skills.
For this role you must have a good understanding of the following:
• Undertaking Medication Reviews and Structured Medication Reviews (SMRs)
• Medicines reconciliation and hospital discharges letters
• Medicines re-authorisation and management of repeat prescribing
• Medicines optimisation and cost effective prescribing

Key Responsibilities
• Process and review repeat prescriptions accurately in line with MMS and GP practice protocols.
• Manage routine medication queries from patients, community pharmacies, and care providers.
• Support medicines reconciliation for new patients and post hospital discharge.
• Assist clinical pharmacists with medication reviews, monitoring checks, and safety initiatives.
• Provide patient facing support on medicines use, inhaler technique, and prescription changes.
• Contribute to audits, safety alerts, and medicines optimisation projects across partner practices.
Essential Requirements
• GPhC registered Pharmacy Technician.
• Strong understanding of medicines, prescribing processes, and legal frameworks.
• Excellent communication, accuracy, and organisational skills.
• Ability to work flexibly across multiple GP practices as required.
Desirable
• Experience in primary care or GP surgery settings.
• Familiarity with EMIS, SystmOne, or other primary care clinical systems.
• Understanding of ICB priorities, QOF, and medicines optimisation frameworks
